Child/Adolescent Appointments
 
When your child/adolescent is scheduled for an appointment, please make every effort to accompany him/her. A child/adolescent scheduled for a physical may need immunizations. In these cases we will need written authorization from the parent/legal guardian. Adult caretakers cannot sign for immunizations. Children and adolescents who received immunizations will be required to wait 20 minutes before leaving the office if they came unaccompanied by an adult.
 
If it is necessary for your child to come on their own, it is important that they have their insurance card with them in addition to the required copay.

IMPORTANT: Connecticut Law requires that parents must receive and review the Vaccine Information Sheet (VIS) for each vaccine your child will be receiving prior to your child receiving the vaccine(s). Therefore, if your child is coming in for a vaccine without a parent and/or guardian, it is imperative that you contact a receptionist so she can give you the necessary information in advance. You can download the appropriate Vaccine Information Sheet for the specific vaccine(s) your child will be receiving from the CDC website. If we do not have this information at the time of the visit, we will need to reschedule the appointment.

After reviewing the VIS for each vaccine your child is to receive, you must complete our own Vaccine Authorization Form, which states that you have seen a copy of the VIS for the relevant vaccines.
